List the 4 principles of Jean Piaget.
1. Children think differently than adults.

What is the dosage of midazolam?

Effects?

What is the reversal?
0.25-1.0mg/kg to a 15kg maximum.
0.5mg/kg has few physiologic effects.

Sedative, anticonvulsant, muscle relaxant

Onset 5-10 min.

Reversal: Flumazenil 0.01mg/kg Q5 minutes then Q20 minutes.

Possibility of resedation. Monitor for 2 hrs.

What is the dosage, effects and reversal of meperidine?
1-2mg/kg to maximum 50 mg.

Narcotic, analgesic, sedative, antispasmodic, REDUCES SEIZURE THRESHOLD (reduce amount of LA)

ONSET:30 minutes

Reversal: Naloxone 0.01mg/kg (disp. 0.4mg/1ml)

CONTRAINDICATED IN ASTHMATICS! INDUCES HISTAMINE RELEASE.
Describe Malignant Hyperthermia
Disorder of skeletal muscle.
Fatal complication of GA.
AD, hereditary

Triggered by volatile anesthetic gases.

Testing: Muscle biopsy

Signs:
increase in CO2
muscle rigidity (masseter spasm, unable to open mouth)
tachycardia
hyperthermia
hyperkalemia
hypertension
DIC (DISSEMINATED INTRAVASCULAR COAGULATION)

Tx: Dantrolene (muscle relaxant), cooling blanket

NOTE: Succinylcholine (neuromuscular blocker, paralytic) may induce MH and severity.
