  • Coma
  • Absent motor activity (including decerebrate/decorticate posturing)
  • Absent brain stem reflexes
  • Absence of ventilatory effort with PaCO2 >60 or 20mmHg higher than pretest

What is the rapid shallow breathing index
an extubation criteria

RSBI = frequency / VT

while breathing spontaneously, if >100 likely OK to extubate

if < 100 likely NOT ok extubate
pt being weaned off vent on PSV. when can you extubate?
  • VT 4-6mL/kg
  • f < 30
  • Good PaO2 and PaCO2
  • PIP 5-8 cm
Diagnosis of ARDS/ALI
  • Exclude cardiogenic pulm edema (PAOP <18)
  • P:F < 300 (or 200 in ARDS)
  • b/l infiltrates on cxr
SIRS criteria
  • Temp <36 or >38
  • HR >90
  • RR >20
  • WBC <4,000 or >12,000 or >10% bands
